DESCRIPTION: It is our Hospice mission, acting as Partners in Care with other community based healthcare providers, to deliver excellent quality care designed to maximize the health and independence of all who qualify for and desire home-based healthcare services in our community. Our hospice is committed to: the concept of hospice care. providing comprehensive, competent care which optimizes patient comfort and dignity. Care that is consistent with the patient and family’s needs & goals, with the patient’s needs & goals as priority. delivering timely end-of-life care that is well coordinated, family centered and includes bereavement & counseling support. consumer oriented case management & individually designed services. ensuring continuity of care consistent with patient, family and caregiver needs as they pertain to the terminal diagnosis culture, environment & appropriate level of care. acknowledging without discrimination the dignity, comfort and choices determined by the patient, caregiver & family including the election of Medicare benefit or alternative health care options. designing treatment protocols & interventions consistent with the mission of the organization. VOLUNTEER OPPORTUNITIES: The duties of the volunteer are to provide companionship and sitting respite with the patient to give the main caregiver an opportunity to shop, visit, or just rest. Volunteers are also needed for bereavement and clerical. TIME COMMITMENT: Minimum 2 hours a week for 3 months. Days flexible. VOLUNTEER TRAINING/AGENCY ORIENTATION: Orientation and patient training in which all aspects and discipline of Hospice is discussed. *Walking distance from the University of Scranton.
